Please make sure that you have reviewed the guidelines before completing the online submission form.

Most of the form items accept regular text, so you can type in whatever makes the most sense to you. For example for phone, you can include more than one number by typing 123-459-7890 (h), 234-567-8901 (w), etc. The format does not matter, as we will have real people reading what you type.

Your summary must be 1000 words or less and include a brief abstract (less than 100 words) an overview, a statement of relevancy to the field, expected learner outcomes, and references.
Remember, the size of your summary file must be less than 1 MB. See the guidelines page for more detail on what you may want to include in your summary.

If for some reason your submission fails, please contact our webmaster. If you submission is received, you will receive additional communicaiton in 6 weeks or less.
